Order Pathway Reticulospinal Interneurons 2nd Neuron Describe Descending

The descending analgesic pathway involves commands from the brainstem to activate interneurons via serotonin, releasing enkephalins either postsynaptically or presynaptically to inhibit the second pain neuron.

Back 1. Hypothalamus or cerebral cortex command fibers from the brainstem reticular formation descend to dorsal horns of the spinal cord as reticulospinal tracts
2. Some reticularspinal fibers 
a. activate interneurons via serotonin
b. interneurons release enkephalins on the 2nd order neuron in the pain pathway to inhibit it (postsynaptic inhibition)
3. Other reticulospinal neurons synapse directly onto 1st order neurons to prevent them from activating the 2nd order neuron (presynaptic inhibition)
